ported criteria of S/LC -cTnI 6.7 or S/LC -cTnT 5.3 (1, 2 ). Taken together, 55 (15.9%) of the 345 samples were positive for autoantibodies to cardiac troponin I or T isoforms. In these positive samples, 10.9% (6 of 55) were positive for both autoantibodies. The odds of being positive for both autoantibodies increased in the presence of either species; in samples positive for autoantibodies to cTnI, the coprevalence of -cTnT was 18.1% (6 of 33), and in -cTnT–positive samples, the coprevalence of -cTnI was 21.4% (6 of 28). The high coprevalence of autoantibodies to cTnT with autoantibodies to cTnI warrants caution in interpreting the significance of a negative or positive cTnT value that contradicts that of cTnI, and vice versa. One should consider that an autoantibody to antigen may have a negative (interference) or positive (stabilizing) effect on the measurement of the antigen, and that multiple autoantibody/antigen pairs may act independently.

Use of cytoplasmic 5'nucleotidase for differentiating malignant from benign monoclonal gammopathies.
Bone marrow smears from 15 patients with multiple myeloma, 15 patients with monoclonal gammopathy of undetermined significance (MGUS), and 15 control subjects were examined for the presence of cytoplasmic 5'nucleotidase (c5NT) in plasma cells. Plasma cell positivity for c5NT (mean and 95% confidence interval) in patients with multiple myeloma numbered 46.4% (38.0-54.8%), in those with MGUS it w...
متن کاملγ/IgG ratio: role in distinguishing monoclonal spikes from fibrinogen.
Serum protein electrophoresis (SPEP) is a standard screening method for detecting monoclonal gammopathies. Presence of fibrinogen, however, can mimic a true monoclonal spike and interfere with accurate monoclonal protein identification. We describe a novel approach for distinguishing fibrinogen spikes from true monoclonal spikes. We classified 600 individual patient samples into four groups: gr...
متن کاملFactors associated with an increased risk of vertebral fracture in monoclonal gammopathies of undetermined significance
Monoclonal gammopathies of undetermined significance (MGUS) have been shown to be associated with an increased risk of fractures. This study describes prospectively the bone status of MGUS patients and determines the factors associated with vertebral fracture. We included prospectively 201 patients with MGUS, incidentally discovered, and with no known history of osteoporosis: mean age 66.6±12.5...
متن کاملMonoclonal gammopathies in chest disease.
Monoclonal gammopathies are found in patients with myeloma, lymphoid malignant disorders, and amyloidosis. They are also discovered in patients with other diseases, and even in healthy people over 70 years.' 2 In such cases monoclonal gammopathies are of undetermined significance, and therefore of little if any diagnostic value. The aim of the present study was to evaluate the monoclonal gammop...

BACKGROUND The repertoire of serologic tests for identifying a monoclonal gammopathy includes serum and urine protein electrophoresis (PEL), serum and urine immunofixation electrophoresis (IFE), and quantitative serum free light chain (FLC). Although there are several reports on the relative diagnostic contribution of these assays, none has looked at the tests singly and in combination for the ...
